If your child resides in 2 separate dwellings the;
- Primary Parent/Carer is required to complete the (above) with their details (Enrolling Adult 1 & 2 sections)
- Primary Parent/Carer is required to provide the Secondary Parent/Carer’s details in the 'Enrolling Adult 3 & 4' sections.
